Francesco at Holistically is committed to providing a safe environment for all his clients free from discrimination on any ground and from harassment, including sexual harassment.  Holistically will operate a zero-tolerance policy for any form of sexual harassment in the workplace, treat all incidents seriously and promptly investigate all allegations of sexual harassment. Any person found to have sexually harassed another will face disciplinary action, up to and including dismissal from employment.  All complaints of sexual harassment will be taken seriously and treated with respect and in confidence. No one will be victimised for making such a complaint. 

I, the undersigned, have chosen to consult with and hereby give consent for massage therapy to be provided by Francesco, having provided a medical history, I do not expect the therapist to have foreseen any previous or pre-existing condition that I have not mentioned. I am aware that the therapist does not diagnose illnesses, prescribe medication or physically manipulate the spine or its immediate articulations. I may understand that massage therapy may produce temporary muscle soreness, occasional mild bruising, increased awareness of areas of pain and some light-headedness. I will tell the therapist about any discomfort I may experience during the therapy session and understand that the therapy will be adjusted accordingly. The therapist understands that I have the right to question procedures used and to receive an explanation of any procedure that they perform. I have read and understood this Disclosure Statement, which remains valid for all my appointments with  Francesco at Holistically.
